Letters

Any letters, phone calls, documentation, assessments, and consultations will be billed per current rate sheet (available online in our knowledge-base https://knowledgebase.kinghealthassoc.com or from admin staff).

Letters will be provided within 21 days of request and phone calls need to be requested at least 36 hours in advance. Any documents or statements longer than one page will need to be requested 28 days in advance. A minimum half-hour payment will be required prior to the provider’s outside-of-session services. Upon completion, you will be electronically invoiced for any additional time that was spent, which must be paid in full prior to the documentation being release. 
Associate licensed providers do not provide letters or documents to clients. Associate Licensed providers do not provide written documentation to third parties. This includes but is not limited to FMLA, SSDI/SSI, TANF, and legal letters, assessments or paperwork.
