If your provider is subpoenaed or asked to appear in court or via call for you, you will be responsible for the fee for the time required of your provider, including travel time and associated expenses, calculated hourly from the self-pay rate (see our rate sheet available online in our knowledge-base https://knowledgebase.kinghealthassoc.com or from admin staff) and must be paid 14 days in advance or your provider will not be able to appear.

We reserve the right to refuse to provide letters or any additional documentation for litigation purposes. For proof of diagnosis or mental health status, you will be able to request medical records and session notes, upon approval. Some exemptions may apply based on provider discretion.

We reserve the right to refuse to provide letters or any additional documentation beyond your medical records, for any reason. This may include, but is not limited to; FMLA, SSDI, or employment paperwork.

Associate licensed providers will not provide legal documents or appear in court. Associate Licensed Providers do not provide court ordered treatment.
