Insurance

If you would like to use your insurance, it is required that you provide your insurance ID and Group number information prior to your first appointment and provide our admin department with a copy of both the front and back of your insurance card at check-in, If you do not provide your insurance information within the timely filing period allotted by your insurance plan, you will be fully responsible for the costs incurred and your insurance will not be billed. Providing insurance information after your session has occurred may incur an additional administration fee.

If your insurance company requires authorizations and/or a limitation of visits, it is your responsibility to obtain the initial authorization and track your remaining visits. Failure to obtain an initial authorization 48-hours prior to your schedule appointment, will result in cancellation until an authorization has been put on file. Standard late cancellation fees will apply.

Note: If you are requesting couples counseling, you must clarify with them if your plan covers it (CPT 90847), as many plans do not.

Although our billing team will attempt to verify your insurance benefits 24-hours prior to your appointment check-in, it is your responsibility to obtain and understand your Behavioral Health Benefits are prior to scheduling an appointment. If your insurance denies the claim for any reason, you are responsible for all charges that have accrued.
